Assessments
Once contact is made with the Leeds Black Elders Association an Initial Assessment will be carried out on the service user as this will determine the needs of the individual and how we can best support them.
The assessment will be carried out by a member of the Leeds Black Elders Association staff who will ask a number of questions relating to your health and general well-being.
Referrals
The Association relies on referrals; these maybe self-referrals; referrals from family and friends or referrals from other service-providers. In turn, we also refer to other organisations and service-providers if the service-user's needs are unable to be met by us.
A member of the Leeds Black Elders team will visit the client in the comfort of their own home to carry out the Initial Assessment. This will highlight any areas which need attention such as:
- Benefits check
- Home adaptations
- Accommodation
- Reducing social isolation
If the Initial Assessment shows that there are gaps in the clients needs, they may be referred to the other relevant agencies but, we will remain in constant contact with the client throughout. This will enable us to maintain our support relationship with the client, giving them a friendly and familiar person to direct any queries or worries to.
Service-users are always welcomed to visit our office without an appointment where our staff will always endeavour to assist to your needs.
