Assessments

Once contact is made with the Leeds Black Elders Association an initial assessment will be carried out on the service user, this will determine the individual's needs.

The assessment will be carried out by Leeds Black Elders Association staff who will ask you number of questions relating to you health and general wellbeing.

Referrals

The Association relies on referrals, these maybe self referrals or it could be from other service providers. We also refer to other organisations and service providers.

Leeds Black Elders staff team will visit clients in their own home where clients are initially assessed to ensure that they receive the correct:

  • Benefits
  • Care Plans
  • Accommodation

To ensure that these services are met they may be referred to the relevant agencies. Service users can visit our offices without an appointment where our staff will endeavour to assist to your needs.
